If you craft Two-Handed Mighty Weapons and then use the Kanai's Cube recipe to upgrade them--you will eventually get one. It's all just luck though, so it may take a couple tries or it might take 100. Thankfully Demonbuddy helps a lot by doing all of the farming work for materials.

Don't buy any weapons, just craft level 70 weapons or armor (yellow) and use those. Or spend your Blood Shards and get a bunch of them that way. After you've done it a few times, it will be simple and you will start to upgrade all of your gear. If you don't know how to craft gear then give up...

You have to restart the game & launcher to download the patch; at least that's what I did--but then Demonbuddy won't work because of the new patch.

It runs on Windows 10 but has the same issues that everyone is having regardless of which version of Windows is loaded.
