What happened to my DB's avoidance?

Trinity used to be really good at avoiding things, now it seems no matter what settings I use, the bot just WANTS to run inside a molten core, or right on a Grotesque when it's exploding. It also just runs inside a huge pack of mobs to fight instead of casting from a distance, on a WD.

Any help as to why it's doing this even with avoidance settings maxed out basically?

Just 2 weeks ago i was comfortably farming gr60's on my WD and now i can barely make GR50 time cause of all the deaths
 
please post a short log that exemplifies the problem, with DB logging set to Verbose in DB settings window and trinity core logging options enabled under advanced.

dying from frozen and running into the middle of packs. It tries avoiding by going right outside of the molten core, frozen, etc and going back and forth in and out until i either get lucky and am outside the explosion zone when it happens, or it blows up that second i'm inside the zone.
 
I currently dont have a log, in the evening I can supply one.
But I mentioned this too. Getting hit by molten core and ice shards very often, even if I raise the dodgerange.
 
I get this problem all the time. Minions that explode after death, my bot ignores the explosion and dies, arcane turrets/beams, my bot likes standing in them.
 
Yes, if there is a setting that can help fix this, please do share. I've lost 10 chars to this in the last week...
 
I agree with this I'm sitting here frustrated. Just leroy jenkins right in the middle of shit. resurects and continues running forward into more shit then dies again. What happened? I'm thinking about rolling back to an older beta build
 
Same issue with my WD although not all of the time. Sometimes it will just stand in stuff and die and sometimes it gets out like it should.
