From the information you provide, its hard to diagnose your problem. But, here are some things it might be...

• You might need to turn on Honorbuddy -> General Settings -> Kill Between Hotspots
• If you're running a grinding profile, you might be headed to the first grinding hotspot, and the mobs you encounter on the way are not in the profile's Factions list.
• There is a known problem (Bugtracker #149) where Honorbuddy frequently ignores mobs when it decides to Repair, Train, Mail, or Harvest.

In short, it could be a lot of things, including Honorbuddy bugs. More detail would help an analysis.
