Tyrael with Demonic levelled from 1 to 81 (So far), no interrupts, no CC enabled, eating disabled, summoning pets disabled.
Around a week + of routine botting playing between 1 - 12 hours a day depending on circumstance. Playing like i normally would. Going for breaks, making tea, walking the dog etc.
Friend of mine has been quest botting for 12 hours remotely and in person and also using Tyrael and he's currently not banned and is 82. Spending more time quest botting then routine botting.
At the moment all I can say is it seems people who have easily detectable things enabled are getting caught.
Personally i think it has something to do with Click to Move and as such i always turn it off as soon as it comes on.

From reading the ban section I've come to the conclusion that a lot of people are being banned for bloody obvious things.
People who botted while doing LFG dungeon runs, I've grouped with a few botters and they are so bloody obvious its not worth the risk personally.
Full botting in PvP, RBG's and so on, this is another blindingly obvious thing you shouldn't do personally, everyone follows the same path, dies the same place etc and it just stands out too much.
Having .8s (800ms) or less reaction time on interrupts when the human average is around 1.5s (1500ms) is another MASSIVE give away, insta totem killing when they spawn too.
Then you have the obvious quest botters, moving in convoluted patterns and getting stuck consistently on objects.
In the end, people who take risks will be the first to be banned, if you protect your self you'll be a lot less harder to detect. As others have said, take breaks, play like you would without the bot, don't AFK bot and go out the house for 20 hours and expect everything to be fine.
If your raiding/PvP'ing remember that not every player is at the top of there game all the time, if your raiding, turn it off for a farm boss and do less DPS and perform badly, this will add to the human feel.
Also the later in the night you play the more tired and distracted you are, so again you should be playing worse and taking more risks, remember this when your rocking DPS meters at 3am.
Botting is becoming harder because there becoming more popular and as such you need to take more measures to protect your self unless your happy to get banned.
