Manglemaw run has been patched: it does not drop any loot.
Good news:
there is a way to let him drop items again.
it requires + 5 seconds.

so you basically run out the door and turn left, when the character says something "feels like the crown is near ... " along those lines, then run back towards the portal door.
when it spawns, kill it and you'll get loot.

Manglemaw STILL doesn't drop any loot if you don't get to the quote fast enough.

Basically only DH/WIZ can do this now..

I see you are in US.
On EU the patch hasn't gone live yet and legendary items still drop.

Okay I tested with my Barb on T4..

You have to Leap to the door on the bottom side of the map and walk a bit towards the door until you get the quote - if you don't Leap to the door you won't get any loot.

I don't think they changed the loot table for this guy.
It's just RNG, hopefully.
