How is this possible?

I have had two exact items drop at the exact same time from the same monster, identified to have the same exact stats, on the same account/character (not across acts or characters). It has happened twice to me, once as recently as last week.

The item(s) was a junk yellow for me in both cases, not lucky enough to get duped legendaries.
