HB facing the target

Seem to have problems facing my target. This started recently it did happen a few times before but more recently it has been happening a lot where the target is just slightly off to the side and HB doesnt adjust.

I leveled a character from level 1 so far it is up to 63 with HB and from 1-60 is was doing ok sometimes having this problem. Lately (after i got into outlands) it has been doing this a lot i have a very high death rate and have the best gear i can get for level 63 (have no problems taking down 4-5 mobs if the bot works right).

What can i do about this? Is there a setting i can change to make it correct this.

Also with another character I am having trouble with A LOT is that when it trys to pull and it says "target not in line of sight" it will just keep trying to pull it will not try to move to correct its position.

1-63 Warrior Default at first then tried ensemble then went back to default because it didnt support fury then changed to arms spec after dying to much and went to ensemble. The problems are with both CC not just one or the other though. Low levels tried a human mage, once i got to the kobolds (level 7 or so) i had to go into the kobold mine and it wouldnt correct the LoS. Created a Gnome mage right at the new starting area for gnomes it does the exact same thing LoS error or the crazed gnomes.
