HB 1.9.3.1 Druid CC Help.

Is there a Druid CC that will, on the last 20 to 25% of health on the mob, switch from Feral to Casting form on runaway mobs so I dont run into a group of mobs to kill the one I am fighting and get waisted, or modify a current CC?

I have tried both of those, chicken is good but it stays in feral, and the other one switches back and fourth too much and dies,,, what I need is one that stays feral, fights to the last 20%, roots and wrath the last part. I am not a developer and do not know the first thing about coding.

If no one can help, can someone tell me How to Edit a CC to do what I need it to do,,,, Feral, Feral, Feral, Ballaanced?
