Getting stuck using only Auto Attacks

My guys will be using all of his attacks one minute, then gets stuck with only using auto attacks till either he dies or moves to the next mob at which point he'll start using all of his attacks again. Didn't seem like it happened terribly often below 68 but now 80% of the time it's only using the auto attacks and keeps getting my guy killed. Any suggestions?

Edit: Now he just charges the enemy and only uses auto attacks, charges the next enemy auto attacks, etc
All of my addons are turned off.
