1. I am aware that I need to load a combat profile for db to know how to and what to fight for...
There are profiles included with the bot. Pretty much most of what you need is already in the DB > Profiles folder.
Most profiles you will run are in the DB > Profiles > Adventurer folder.
...but in the profile section on this forum, why it's listed under Act1 to Act5? Aren't it suppose to be builds for Wiz, builds for Barb? Or more specifically, WOH wiz profile, or HT WD profile?
It sounds like you're confusing Profiles with Combat Routines.
Profiles are what you load so that the bot knows where you want it to go in terms of traveling and game mode.
Combat routines control the combat and ability usage. Combat routines for different classes are already built into Trinity.
Trinity looks at the skills and/or gear you're wearing and determines what build you're using. It will approach combat accordingly.
You can also modify how certain skills or avoidance are activated and handled in game via teh Trinity Config menu in DB.
Here is a guide on what all of the settings in DB do:

I found a thread give me all act quest profiles, but what's that already included in the latest db build?
Just use what comes with it unless there is a newer version posted after the DB release came out.
Why would you want to use the Questing profiles? It completes the normal game and is sub-par for leveling, powerleveling, gearing and general game progression.
Any of the profiles in the Adventurer folder would be better for any goal that you might have.
2. what controls this bot what to do? Like if I want it to do quests or Grift or bounty runs?
Load the appropriately named profile that is found in the DB > Profiles folder

3. what's the difference between Trinity and Quest tools?
For 99% of things you would no longer need to use QuestTools. If you have to ask, you don't need it
4. I saw a lot of item rules here on the forum. but I also saw a lot of settings in db itself like if I pickup this or that. what's the difference?
Use the ItemList found within DB itself. You can create rules on your own or import lists that people post on the forums.
