DB frequently ignores loot

Can someone help? I don't know why DB sometimes WILL loot and sometimes will not even if I use the same profile/loot rules.

Currently running Queen's rinse repeat (hell) with default loot rule and radius of 40...well my barb hasn't looted a single piece of gear. I've tried adjusting the number but that doesn't seem to have an effect.

Community member Giles has stated and proved many things wrong with the DB default rules; or looting 'laws' period.

Not sure if this is something detected, but best trying to find his posts; as there is A LOT of good, useful information in them.
