D3 freezing, DB remains open

I've noticed my D3 keeps freezing at random times. D3 will freeze (not responding state) but DB will remain open.

The last line in the DB console is always:

========== Grid segmentation resetting!!! ============

I have done a clean re-install and DB twice, and a clean install of .NET 4 and C++ redist.

It still seems to be happening. See attached LOG.

My computer specs are:
i7 3930K
16GB ram
Radeon HD6900

Looks like it might be a problem with Trinity.
See if you can find an update done by the community and it should work.
 
This happens to me on an AMD A series APU laptop but only in random dungeons.

This doesn't happen all the time.

It seems as thought DB can detect my game has frozen:

I just can't find the cause. I re-installed trinity with latest versions. I've used old versions and everything, I've had trouble with this for a while and finally decided to post. See my two newest logs of this happening.

It doesn't happen straight away, sometimes the bot can run for a decent while (few hours) before freeze. I have both DB and D3 on admin rights.

A few things I've noticed that might cause it to freeze, sometimes the game "lags" like freezes for a split second and comes right. To explain this better I will stop for a tiny instance then I will run very fast to make up for the time.

What is even more bizzare about this, I run 3 instances. Generally 1 instance will always be stable only the other 2 will crash.

This is about all the information I can give you so far, any more ideas?
