D3 Crashing and Poor Performance - Common causes and possible fixes

I see this problem is back. It appears to come and go. There is a significant lag/delay when connecting to the DB servers from the client too. I have a 25Mbit connection, usually _very_ fast when connecting.
It now takes up to a minute to connect.
