Authenticator makes the difference? Seems difficult if it gets disconnected/using with HB relog.

People got banned with Authenticator as well. Makes zero difference. Once they spot the difference between the original name and the new one , you are done.

Maybe they were reported. I just can say it helped me to avoid the "that's-not-your-account" banns. But I don't used the accounts very long. They got banned too for what I did with them (duping, exploiting, etc.). But that was the only way for me to keep them longer as two days alive.
Or maybe they need longer to bann accounts with autheticators on it. I didn't test, because I didn't need them very long (all what I needed them for was fixed very fast).
