Hand leveled partly(Probably to level 10 or so), Quest bot rest of the way.
5) What combat class were you using? if None Just put Default then the Classname:

I think I was using GilesTrinity, RadsAtom and possibly Unstucker.
7) How many hours per day did you bot for?:
About 12 hours
7.5) What honest percentage of time did you supervise your bot?
Dual monitor setup, I always played CS:S while watching the bot go on the other screen, so all of the time, 100%.
8) How many auctions per day did you have?:
Max, but they were really just selling low end stuff really that any player could obtain.
8.5) Did you use the Real Money Auction House (RMAH)?
Nope.
9) Did you Use Any Other Bots, Hacks, Mods, or Exploits?
Nope.
10) Was your account involved in gold selling?
Nope.
11) What server region where you connected to (The Americas, Europe or Asia)?

So this isn't "recent", but I thought I'd share my experience. It was whenever Diablo 3 was on sale for $40 or something, I think around Decemeber(?) and I got my character up to level 60 fairly quick. When I started botting on the account, I was running the same profile and setup I had on my other accounts (they weren't running at the time though). After a few days of going, I noticed I had been banned when Blizzard did their massive ban sweep for whatever reason.
Now I tried petitioning this ban a good 7 times since I kept getting copy/pasted answers, until the 7th and final time, the person snapped at me and simply told me he had detected a bot, whatever. Thus I went to Paypal and petitioned it with them to get my money back (since it was less than 30 days of the purchase and I was "wrongfully" banned). Couple days went by and I was given a message that was sent to me and Blizzard about this petition and that they'd have to give a reason to me, Blizzard never did and Paypal gave me my $40 back from Blizzard.
And that's how that went for me, fun times eh?
