I will post my set up but I don't think you should follow it to the letter, some of my gear was set up specifically for Ghom, some not. Also, my aim was never to explicitly do as many GPH as possible; it was to maximise LPH (obviously before i realised the error would cripple my LPH, i tend to 24/7).
Helm -
MOJ (+100, I don't have max crit, Flawless Topaz for extra LPH since the others are pointless*) *Assuming Mara.
Neck -
Mara's (Trifecta, not maxed rolls, but decent.)
Shoulders -
Aughild (+Dog , rolled for when i actually play, not Ghom, only 13%)
Bracers -
Aughild (+20 Fire/ 6 CC)
Chest -
Cindercoat (only +16% fire, but +15 Dog, again, rolled for when i play)
Gloves -
T&T (Trifecta - 48 - 9CC/48CHD/6IAS)
Belt -
TWH (7/48/500, basically flawless)
Ring 1 -
TMF (Trifecta 6/46/5.5, very good but not perfect)
Ring 2 -
RoRG (Trifecta 7/49/6, essentially perfect, used to activate Aughild and to increase my MF/XP /w Cain's)
Legs -
Cain (Used for 2+3 set bonuses)
Boots -
Cain (+Firebats, only 14)
Weapon -
Sunkeeper (+30, 2.1k, Flawless Emerald)
Offhand -
Shukrani's (9.5 CC/+15 Fetish, it's a nice offhand i'll probably use for something if SMK ever drops so not rerolling the FA dmg)
Follower -
Furnace (+6%, Flawless Diamond),
Moonlight Ward (Trifecta +25%(No idea if her "normal" attack is classed as arcane or not, the graphic looks like it might be but that's no guarantee!) 7/98/9, i don't think this is the best amulet -
Halycon/Overwhelming Desire might be better, as noted by you guys),
SOJ (+25% IAS, no element since it was merely a "spare", if her regular attack is arcane then the goal would be +arcane/+30/+IAS),
Nagelring (IAS/50% MF extra offensive stats are gravy,
a +elite damage ring like unity or a double IAS rolled oculus will probably both trump it in terms of pure GPH),
Smoking Thurible (Unless i'm forgetting something Enchantress only has 3 skills that could conceivably increase your damage, so focus doesn't particularly matter - obviously
if she dies without it then Thurible is clear BIS.)
Skills - I doubt much of this is any different to anyone else's, but posting just in case.
Vampire Bats - Hungry Bats (Highest damage, plain and simple.)
Zombie Dog - Burning Dogs
Gargantuan - Wrathful Protector
Big Bad Voodoo - Slam Dance
Fetish Army - Tiki Torchers
Obviously I have selected these because they benefit from + fire, I'm not saying fire is the best. Frankly I don't know, but when life gives you lemons...
Spirit Walk - Umbral Shock (If i'm using Shukrani's I may as well take the only rune that could potentially do damage to Ghom*, haven't watched the bot to see whether this ever happens. Benefits from + fire as an added bonus.)
Passives are probably locked in and don't need any explanation.
Stats - 1.85M sheet/ 60%CC / 53%IAS/ 509%CHD/ 2.4% CDR (paragon only)
Paragon 600+
