if you're using 4245, check post 2 of dicussion thread and download the default quest behavior files.

copy and paste them (and over write) them into your current quest behavior folder

Here it is, i did download the new Quest Behaviors, still the same error.

Yeah, I've done it a few times now and restarting both WoW and HB.
It was working fine, until this one area. No idea what happend.

That's a quest behavior of his, but he shouldn't do it that way... open his profile and search for "27388" and you'll find that behavior.
