Updating the suspension/ban form?

I sent CodenameG a message but he hasn't replied, maybe he's busy, whatever.

The ban form could really benefit from having the AH/RMAH fields removed and others included, such as if people were members of a clan, if they had other players on their "recent players" list, approx what their elite/monster kills were, what their paragon level was, how much gold they approx accumulated, etc.

D3 is a different game now that RoS is out and there's a lot of useful information that could be provided other than profiles and plugins used, and how long people ran the bot for per day.
 
I think a form like this makes a bit more sense:

1) What server region where you connected to (The Americas, Europe or Asia)?


2) Were you using DemonBuddy on the account at the same time the ban occurred (kicked offline due to being banned) ?


3) If not, when was the last time you ran the bot?


4) How many hours per day did you bot for?


5) What percentage of time did you supervise your bot?


6) Which profiles were you using and what were your average number of games per hour?


7) Which plugins were you using?


8) How did you level your characters (3rd party service, profile, by-hand, etc)?


9) Were you a member of a clan? If so, how many members did it have?


10) Were their players on your "Recently Played" list who may have seen you botting?


11) What paragon level was your account?


12) Approximately how many elite kills and monster kills did your account have?


13) How much gold would you estimate you accumulated by botting in RoS?


14) Did you use any other bots, hacks, mods, or exploits?


15) Any additional information?

its not as easy as just changing it. the questions where designed for our developers and staff to find issues with the bot and spot possible problems. not so the community can speculate on if maybe blizzard is looking for something or some jerk is going around reporting people.

at the present time no changes to the ban report template will be made.

i will however try and talk to our extremely busy developers to see if there's any fields they need added.
