questions about flagged accounts and 72 hour suspensions

One of my accounts was hit with a 72 hour suspension but after the 72 hours were up, I was able to farm 24/7 for over 2 weeks until my game time ran out. That account was never banned and is still frozen today. During this ordeal I had only one account running on my entire computer which was the account that was suspended for 72 hours but never banned. So just to make a few things clear, are all 72 hour suspensions player reported? Do GMs give a 72 hour suspension without player reports? Do player reports lead to a perma ban on the first time? Since this account was hit with a 72 hour suspension, it must have been players reported right? Is your account flagged after player reported 72 hour suspensions or is your account flagged after it was hit with a 72 hour suspension regardless of player reports or without player reports?

Noone know for sure what they can track and how they do this. But as I've done research someone said that there is an autoflag if you are logged in for 12hr+ without logging out. But account's is flagged basically because of player reports.
