Plugin to switch CC without closing HB?

I thought I remember seeing something like this once before... See the issue I have is this; When running my druid in IB he will sometimes switch to bear form... This looks very bottish and causes the tank to ragekick me all the time... But if I turn off bear form all together, while he is grinding and waiting for a LFD to pop he dies more often which means he has to travel all the time to the repair vendor.

If we cant get something like this... does anyone have a config for fps druid that works great for IB&grinding both... or know which other CC works?
