Phalanx Crusader not casting on small groups

Could someone please advise me what tweaks I need to make to cause my crusader to always cast phalanx? I changed the settings for it in Trinity with no luck. He'll stand and regular melee hit single targets despite being full wrath and significantly slow down kill speed.

Also, in grift trials he doesn't use phalanx at all.

put phalanx on left click and put another cooldown skill on right click (ex: provoke)
it works
