1) Were you using DemonBuddy on the account at the same time the ban/suspension occurred? (Kicked offline due to being Banned)
No, I was offline

2) If not, when was the last time?:
Roughly an hour and a half before the ban, I received an email which notified me of the ban (rather than signing on and finding out)

3) If using a public profile please name it here:
I was using DBTag Trinity (practically started this profile the same day of the ban, originally I was using Big Red's champion hunting a3)

4) How did you level your characters? Please give some details
Quest tools 1-60 bot leveling

5) What combat class were you using? if None Just put Default then the Classname:
GilesBlank, for a barbarian

6) What plugins are you using?:
Giles Trinity 1.6.3.4, RadsAtom

7) How many hours per day did you bot for?:
roughly 12-16

7.5) What honest percentage of time did you supervise your bot?
20%

8) How many auctions per day did you have?:

8.5) Did you use the Real Money Auction House (RMAH)?
No

9) Did you Use Any Other Bots, Hacks, Mods, or Exploits?

*Note: I have two theories as to how this account got banned.
Theory 1: Banwave
Reading these forums, it seems people are speculating that there was a banwave. This is very likely to be the case. If so, then it doesn't matter what I was doing - I got caught.

Theory 2: Azmodan
I've been botting solidly for approximately a month. I used Big Red's a3 champion hunting profile which did not hunt and kill Azmodan. However, today I began a new profile DBTag trinity which did hunt Azmodan. I was watching the bot fight Azmodan and he kinda "tards out" in order to dodge the bloody pools. Considering in the video: Diablo 3 - Billion Gold Challenge, Auction House (part 2) - YouTube blizzard apparently admits (on the account of Moldran, not blizzard themselves...) to using algorithms to detect automated activity (via AH, gameplaying). I think one of their bot-finding algorithms looks up how Azmodan is fought. It's the only new variable introduced to my botting procedure.
