Need a web based submitform for applications

Hope someone can give me some tips regarding this.

I need a web based form for applications, where I choose fields that the applicants should fill in. Also, there must be a admnin login where I can check what to search for. This is to be used as a CV database.

Anyone have suggestions?
 
Those forms are easy to script yourself hehe :-)

But my suggestion would be to script it yourself ^^

i believe there are some good opensource webforms, but i haven't found one (maybe i'm to stupid to use google)
but i know there is a good, and easy to configure one implemented in eqdkp (i'll not provide a link, i duuno but i think it's against the forum rules^^, google it)

But like the last poster said ... they aren'T very complicated, try to write one by yourself :) ... or try googling (with the keywords webform application u'll find nothing^^)
 
I don't know if this is too simple for you but I use Google Docs for this. Create the fields you want in their spreadsheet and then choose the option to make a form out of it. You can manipulate each input to look how you like it (checkbox, dropdown, fill in, etc...). All the info is tabulated into the spreadsheet. Maybe you could then import this to what you want? I don't know. Its what came to mind to me cause I have no coding skills to make my own.

Right now it is real messy and hard to sort. Would be much better to have a GUI that let me filter the different items in line 1, like name, adress, profession, years experience etc. Check ImageShack? - Online Photo and Video Hosting
