Most likely they're using more than one single method.
Also blizzard is filthy rich. They could pay 1-10 people to spectate random people at high paragons and still catch alot of botters. Realistically most D3 licenses are either inactive or not above 600-700 paragon so getting one botter per day each would on it's own give an impact.
Not really banking on this, or any other theory but meta-data analysis and the gap effect between casual and botter/actual hardcore players is growing.
In my mind you have an unknown fraction of a percent chance of being busted. And just like being hurt in a car crash would really suck and manypeople fear it or just accept that that is the risk of driving/being in a car it's not gonna happen to most people in their entire lifetime.
Though doing stupid things in traffic will increase your chances of being hurt drastically, being safe gives guarantee either.
This applies to botting aswell. If you are the highest paragon level in the world you can't hide it and blizz will take a look at some point, still people have botted for short and randomly timed periods and gotten banned.
