Banned, and I Speculate Everyone Here Will Get Banned, Sooner or Later.
Just got banned today, can't access data from server for hours (checked with official game app, got the stuck screen message), so likely a perma.
*Note: After getting the error for about 3 hours, I created a new account and started it up on the official app on my phone via the same internet connection (Wi-Fi) as my bot (my PC), which, to no surprise, could access the game normally, while the botted account still couldn't.
Therefore, it is a pretty solid-proven ban.
My way of Playing:
0. Level at the time of ban: 27
1. One account ONLY.
2. Never had Botting session for more than 6 hours straight.
3. Never botted for more than 12 hours per day and always took breaks from botting when eating, sleeping. And the eating breaks were usually longer than 1.5 hours.
4. Never went above jogging speed.
5. Never sent bot to more than 15km(radius) from my home.
6. Always waited for at least 30 mins after terminating the bot to access Pokemon Go on my phone / ipad.
7. When shifting (mini-teleport?) bot from one place to another (still within the 15km radius from home, and usually shifting less than 10km each time), always waited for reasonable traveling time before re-starting the bot, depending on distance. Additionally, I never "mini-teleported" more than 5 times per day, usually less than 3.
In short, the way I botted was like a regular, dedicated/hardcore player, who would play all day except eating and sleeping, while exploring my home town.
I suspect not many botters are botting more cautiously than I did, so, good luck. It is very likely you would not get away with botting, just the time has not come (even with super computers, it takes time to scan through all log data from millions, if not billions, accounts), unless bot-devs truly figured out the methods/criteria Niantic is using for anti-botting/spoofing.

Btw, not checking medals/achievements, confirming level-up inventory bonuses and hatched eggs surely are blatant evidence of illegitimate play.
If these are not implemented, Pokefarmer will never be a safe bot to use, IMO.
