How do I know if I am banned? And how do I know how long I am banned for? Also what can I do about it?

"failed to get data from server" - 99.9999% you're perm banned, or you can wait for the official email sent to your email account confirming the ban.
Your account is banned FOREVER - not joking
You can appeal un-ban request - However, I doubt it will works. If you actually botted/hacked, you're fu**ed, no way of getting it back, Niantic doesn't give a shit about your account.
