This happens when armor is damaged enough to repair. It does not matter whether the GR was completed in time or not. Bot ports to town, does the repairs, sometimes does other things too, then ports back to Urshi. Figures out what its going to do about Urshi (upgrade gems or ignore her ignoring it since it failed), finised gem upgrades if appropriate, then ports back. I'm not sure I have ever seen loot lying on ground to be picked up. I know vast majority of time there is not.
Obviously dispensing with Urshi and foregoing two unnecessary ports would be a lot more efficient.
On a similar note: WHY does bot decide to use the Book of Cain, RUN over there, then ALWAYS wait seconds before actually opening the Book? No idea what its "thinking about", but surely that could be done while the items are being identified?
