make a new HB installation from scratch and dont use any 3rd party stuff
test it and let me know

it happens only on new dungeons?

it's not only dungeons, this occurs most often in ghost form at (Hellfire Ramparts)!
when my characters will train 58 level, I will try what you told me

i'm download a new HB at the site, and after death at "zul'farrak" in ghost form my character going around the hot point again,
the duration of the action 10-30 second, but this is enough to fall off a cliff in BC dungeons
