Every ~1 hour my bot stop working saying "Grid segmentation resetting"

I'm using Act 3 Champion farming profiles with GilesTrinity and RadprofileManager as plugin, but I have ========== Grid segmentation resetting!!! ============ in DB log chat and my bot simply stop working either in town or in resume screen.
However DB leaves the game and resume while I'm in another map like core of areat not in the TOWN even though I have ========== Grid segmentation resetting!!! ============
This always happen when i'm in town(especially after repairing gears because of the high death count) or in the resume screen(because of high death count, it leaves the game and stay in screen)
What should I do to fix this?
Is there any plugin or any type of program that can solve this problem? or Is it like DB problem?

- Giles Combat Replacer or Giles Trinity with
- Belphegor?
