Using Kicks profile, been botting 'till level 19, then this happened. Could be the profile, I'm also using The Saints CC instead of default singular.

Try completing the current quests that are on your Quest Log, then restarting the bot. This may, or may not solve the problem. Let us know.

It fixed the problem, because the quest itself was causing the problem. It was to do with the bot not reading the quest.

i just quested from 75-85 and this happened probably 10-15 times out of 500+ quests - in some cases it also tried to pick up quests that it wasn't eligible for, so I either completed the quest manually or went into the quest profile and commented out the offending quest and behaviors - over all though it's pretty freakin good - it's far from one click 1-85 but if i had to guess it requires less than 5% effort. I've been meaning to attempt to level another character from 1-85 and see how long it takes.

I started using HB AFTER the 5.04 patch so I don't know how it was BEFORE, but was it ever flawless on questing?

I don't think you could say anything is flawless but pre-5.0.4 Kick's profiles were damn close. I've used them on multiple toons and still do. With all the changes to WoW and HB his profiles will need some re-polishing (but they are still pretty good!). He was waiting for a more stable questing bot first. I think he speculated starting end of the month but don't quote me on that. For now, an occasional assist may be required.
