Character using LMB skill very few times

My Crusader is using the LMB skill (simple attack) very few times, gives 2 hits, and waits doing nothing like it was waiting the cooldown of the other skills.
Do you know why?
Maybe I've setted something wrong?

I removed the old quest tools and trinity plugins and fresh installed with the following ones but there seems to be no change.

Maybe your build is not supported yet, I suggest you to report it to the Trinity forum with the explanation of what you require and the build you use (Diablofans build link would be nice).
