[Beta] Unable to teleport after death?

An issue someone reported to me while using one of my profiles that appears to be related to returning to a zone upon death if your Home Point is set elsewhere:

died on my way to MSQ: The Gift of Eternity

came back with that error and ended up with gear checking spam in the Gold Saucer

I have observed this personally while testing my HW questing profile, but could not locate the log it occurred in. I do know that I died in Coerthas Western Highlands and my Home Point was set to Camp Cloudtop when I got the error. Falcon's Watch Aetheryte was known, but still did not handle a teleport to Falcon's Watch to get my character back in the right zone.

Without logs and the profiles in question I can't do anything. I'm very busy and don't have time to play investigator, least of all making sure the issues reported werent already fixed.
