Ban Reports template?

I was just wondering if we, as a community, can come together for a bit and create a template for the Ban Reports section that we can all agree to use. The Ban sections of this forum is a royal mess, and this isn't the moderators fault, it's our fault. We are posting a lot of "reports" without including any useful information, and we are posting reports and discussions in sections where they don't belong. This causes clutter which in turn lowers the quality of the forum. We are only punishing ourselves because the clutter makes it hard for us users (and devs) to find relevant and important information. So my plea to you, the community, is that we get our shit together and create a template that actually conveys something useful, instead of filling the forum with one liners like "Got banned, GG Niantic". Take a moment and compare the PokeFarmer Ban section with other ban sections on this forum and you'll see what I mean.

As a draft I'm thinking the bare minimum needs to be an approximate account creation date, account level and type of ban. We should also include what version of PokeFarmer the account was using. Here's a few things that I believe are necessary to form a proper ban report, along with the reasoning for including them:

Account
Account creation date: Just so we know how old the accounts are. It will also help us determine if older accounts more resilient than new ones.
Account level: Are accounts especially vulnerable at a given level range?
Type of ban: There are currently 4 different types of bans, with certain bans having more variations: Soft Ban, Hard Ban, Soft IP Ban and Hard IP Ban. This may reveal a pattern i.e. "X amount of type soft bans leads to type of hard ban" etc. Perhaps even a time stamp from the log to help detect ban waves?
PokeFarmer version: Are some versions more detectable than others? New update may have introduced a detectable security risk? Unlikely, yes, but either way it will help the developers to quickly react if the situation arises.

Usage
Active hours per day (average): This will help form a picture of the general usage, and separate the "suicide" accounts from the "hardly ever used" ones.
Has account been used with any other software? (BlueStacks, NOX, etc.): Perhaps a certain software poses a bigger security risk than others?
Proxy/VPN: Specifying if the account used PokerFarmer's proxy settings, or a third party VPN service, may help to detect IP leaks or other security risks. Perhaps we have been using shitty VPN providers?
Other: In case user has comments, or wants to provide additional information that might be relevant to the report.

That's what I have for you on the tip of my tongue. I'm open to any changes or additions to this as long as it doesn't contain any identifying factors (usernames, IP addresses etc). For those of you who think that this will be to much to fill out after a ban: The section is called "Ban REPORTS", not "Ban WHINES". Lets at least try be a bit constructive when shit hits the fan.

I know we disagree and bicker about a lot of things, but I'm hoping we can at least agree upon that the Ban section is important to keep clean. It's there to keep our sorry asses from being banned so we could at least show it some respect.
