All that will be shown below is just my speculation.
Since I started use this beautiful product(HB), almost every day I check the HB Ban section for information about ban waves. After a considerable time spent in this section, I noticed that a lot of people banned after GatherBuddy bot system usage and GB2 profiles.
There is a lot of reasons why people get banned, some of them is obvious:

Yesterday I was trying to figure out why, even those who play very carefully get banned. 5 years ago I played game Ragnarok Online and they implement very cool system against botting - Anchors. This Anchors are items\objects which invisible for players, but bot see them and pickup\use. This items were very tasty for bot so they use\pickup them. In WoW I also saw situation when players\bot pickup invisible objects so if bot see HERB in popular location he pick it and then Blizzard see that %playername% picked tagged\anchored HERB and you got ban.
Personally I dont use GB2 at all just because I aware of bans. I use questing\tyrael\bg\ a lot and no bans at all.

There is a lot of reasons why people get banned, some of them is obvious:
- 24\7 botting
- putting a lot of materials to auc
- botting in popular places
- botting in one places with one routine
- reports from players^
Some of them not really obvious:
- simultaneous botting with 2+ windows from one HWID - this is my idea that this is risky to launch more than one ACTIVE window. By ACTIVE I mean situation when 2+ HBs manage 2+ WOW windows from one IP and 1 HWID. I am not specialist but it can be monitored somehow.
- botting while playing other blizzard games - I dont think its possible but this new battle.net desktop app can present surprises.
- etc.
