I have got the coin achievement on every character I have standing at the fountain and fishing, never had a problem, hiding in plain sight :) I have only ever caught the rat twice though, first time took me 3 weeks of fishing 2 hrs a day, 2nd time was the first cast into the fountain :)...

Just fish in the fountain at Dal, you get the coin achievement and also you can fish up sewer items due to a bug in the game. Stand in the far left corner if you were facing Marcia, cast towards the pavement/roadway.
