I think you are on the right track it sounds like a windowed versus full screen issue. Have you tried deleting the settings completely?

That sounds more like a profile issue, try using a basic profile and see if it attacks.

Did you switch to concentration? If you change that file you can switch back to combat if you prefer, and it will work. This fix will go in to the next release of BW
