1) Were you using DemonBuddy on the account at the same time the ban/suspension occurred? (Kicked offline due to being Banned)
I was not present during the time of the ban, but I had my bots running when I left for work and now they're banned so I will assume yes.
However, I had 1 account I had previously botted and quit botting a few weeks ago and that account survived!
2) If not, when was the last time?:
Roughly 2 weeks ago.

Hired a guy to power level them via exp gear and following a runner etc.
5) What combat class were you using? if None Just put Default then the Classname:

7.5) What honest percentage of time did you supervise your bot?

Not many. Mostly traded unids for igg.

On one account, yes.

I occasionally used TurboHud while powerleveling on the account that isn't banned so that isn't really applicable.
10) Was your account involved in gold selling?
On the account with my paypal linked, I had previously sold a few billion or so. Nothing outrageous. Previously, I stopped selling RMAH.
11) What server region where you connected to (The Americas, Europe or Asia)?
Americas.
